TANDEM365 will soon celebrate seven years of service in April 2021. We continue to innovative and challenge the complexities of healthcare, so that lives can be enriched and transformed. We are serving 1150 participants today. TANDEM365 consists of two components, care management and community paramedicine. Our goal is to prevent unnecessary emergency department visits and hospitalizations by understanding the complexities of healthcare and supporting individuals who have unstable health conditions and are in a trajectory of decline.

TANDEM365 is a complex medical case management program coupled with one of the most robust community paramedicine programs in the nation. We receive referrals from insurance carriers and anyone in the community, including PCP offices, hospital discharge planners, family members, etc. We currently serve patients on the program in Kent, Allegan, Kalamazoo, Montcalm, Newaygo and Ottawa Counties.
TANDEM365 utilizes highly skilled RNs, MSWs, and Integrated Community Paramedics to assist frequent utilizers, medically complex individuals, and frail elderly patients to remain in their homes and manage their conditions more appropriately. Our model provides intense, high touch, patient centered methods of care delivery to assist our patients to better navigate the fragmented and costly healthcare system more successfully. Our services can include fall alert pendants, telehealth, chore-worker services, personal care assistance, electronic medication dispensers, transportation, and medical assistants that can fill med boxes and med dispensers for those that are unable to manage independently.
In addition to the case management services, our Integrated Community Paramedic team serves as a rapid response to any non-life threatening, but urgent medical issues that may arise in this patient population. We perform lab specimen collection, nebulizers, oxygen therapy, IV fluids, IV medication administration, EKG’s, lift assists post falls at home, and any generalized medical concerns that do not warrant use of the ED. This service has assisted TANDEM365 to save costs and decrease overuse of the already strained hospital systems and supports our mission of helping patients to age in place.
95% of all TANDEM365 participants report being satisfied with TANDEM365’s services and all the staff that enter the home. Patient satisfaction surveys are sent out quarterly and have consistently resulted in satisfaction ratings at 95% or higher for all 6 years of TANDEM365 caring for patients.
During the onset of COVID-19 it was recognized that TANDEM365 could potentially provide support and interventions during the crisis. We signed a contract with the hospital system to assist with patients of their Home Based Primary Care Program. We also signed a contract with Hospice to assist in after-hours in-home support during a staffing crisis due to COVID-19.
Our community paramedicine team has also been asked to assist in the administration of BAM (bamlanivimab) therapy which is a neutralizing antibody treatment for COVID-19. The first clinic was held at the VA here in Grand Rapids for several veterans infected by COVID. The other clinic was at a retirement community for residents infected by COVID. Our team was there to support nursing and provide onsite emergency support if needed. It was an honor that we were chosen in this groundbreaking event and potentially being the first in the nation to be involved in this type of therapy in these settings. The two events served over 50 individuals and prevented hospitalization for this treatment.
We are proud of the achievements we have accomplished during this very difficult year and believe that each of these situations where we were able to step up, will lead us to something greater. We are also pleased to announce Michigan State University is in the process of publishing our outcomes, which strongly indicate that the services we provide save the health system millions of dollars. These publications will further our stance that community paramedicine and individualized care management make a great team in reducing healthcare costs and improving quality of life.
Further develop and expand Virtual Case Management while creating collaborative education, patient engagement and expansion to remote territories
Further develop the lean management process to become more effective business by eliminating wasteful practices to improve efficiencies. The end goal is to deliver value to our patient population with minimal waste
